1. Application Form for Direct Banking - Personal                                                           

Fill-in, print the application, sign it and send the Original Form to: Bank of Cyprus Direct Banking 97, Kerinias Avenue, 2nd floor Platy, Aglantzia P.O Box 21472 1599 Nicosia, Cyprus Or hand it into any Bank of Cyprus Branch (Preferably to the Account Branch)

2. Application Form for Direct Banking - Company                                                         

Fill-in and print the Company Application AND Company Resolution. Sign them and send the Original Forms to: ΝΟΤΕ: Application for Companies and resolutions must be signed by all Directors & Secretary and stamped with Companys' seal. Bank of Cyprus Direct Banking 97, Kerinias Avenue, 2nd floor Platy, Aglantzia P.O Box 21472 1599 Nicosia, Cyprus Or hand it into any Bank of Cyprus Branch (Preferably to the Account Branch)

3. Application Form - Digipass
